Upload a document. MedStory reads it and pulls out the medical details — test names, values, dates, diagnoses, medications — and turns them into structured data. That data lands in your personal vault, organized by date and by type, and you can search or filter it instantly. If you're managing care for a parent, partner, or child, you can link their vault to yours so their records show up alongside your own, without merging the two into one account. If it's medical paperwork, MedStory can take it — lab reports, radiology/imaging summaries, prescriptions, discharge summaries, vaccination records, consultation notes — as a PDF, scan, or phone photo. You don't need to convert or clean anything up first. Handwritten prescriptions and slightly crooked phone photos are expected, not edge cases.
The document is stored securely in your vault the moment you upload it, exactly as you gave it to us — the original file is always kept, so you're never relying on MedStory's interpretation alone. From there, it's queued for parsing.
MedStory's AI reads the document and extracts the medical content — test names, results and units, reference ranges, dates, diagnoses, medications, dosages — and converts it into structured, chronological data. A blood report you uploaded becomes a set of values you can track over time. A prescription becomes a medication entry with a name, dose, and date.
MedStory doesn't depend on your provider using any particular digital system. It works directly from the document you have, regardless of whether it came from a large hospital chain, a neighborhood clinic, or a lab you visited once. If you can get a PDF or take a photo of a printed report, MedStory can read it.

A lot of health record-keeping in India happens on behalf of someone else — a parent, spouse, child. Each person keeps their own individually-owned vault; you can link a family member's vault to yours (with their consent) to view and help manage their records from your own account. Linking is reversible and visible — not the same as merging two people's medical history into one account.
